A baby born at 32 weeks and a baby born at 40 weeks who are both "4 months old" have not actually had the same amount of time to grow — one of them started the race two months early. Standard growth charts and milestone checklists assume a full-term start, so for preterm babies, pediatric practice reads them against corrected age instead. Here's what that means, how it's calculated, and why it turns many scary-looking charts back into normal ones.
What corrected age is
Corrected age (also called adjusted age) is your baby's age counted from the due date rather than the birth date — in other words, the age your baby would be if they had been born at full term. It answers the question the growth chart is actually asking: "how long has this body been growing since term?"
The usual calculation, as described by the American Academy of Pediatrics:

So this baby's weight belongs on the chart at the 4-month position, not the 6-month one. Plotted at 6 months, a perfectly growing preemie can appear to be far below the curves; plotted at corrected age, the same measurement often lands comfortably in range. The correction isn't flattery — it's removing a bookkeeping error.
How long correction is used
The commonly cited practice, per AAP materials, is to use corrected age for growth and development through about the first 2 years, by which point most children born moderately preterm have caught up and the difference stops mattering. Some follow-up programs correct for longer for very early arrivals, and some stop earlier for late-preterm babies — another thing that varies with your care team's judgment.
Two ages, two jobs
Corrected age doesn't replace chronological age; the two answer different questions:
- Corrected age — growth charts, motor and developmental milestones, sleep and feeding expectations.
- Chronological age — immunization schedules, which generally follow actual birth date, and routine visit timing.
This split is exactly where mental arithmetic fails at 3 a.m., and why writing both ages down — or letting software do it — prevents a lot of double-checking.
A worked example at the checkup
Suppose your daughter was born on March 14 at 32 weeks, with a due date of May 9. At her mid-September visit she is six months old by the calendar, weighing 6.1 kg. On the 6-month girls' weight-for-age chart, 6.1 kg sits low — the kind of position that starts a worried car ride home. But her corrected age is roughly four months, and at the 4-month position the same 6.1 kg lands solidly mid-range. Same baby, same scale, same day — the only thing that changed is asking the chart the right question. This is why it's worth confirming at every visit which age was used to plot, especially if you're comparing the clinic's numbers against your own records.
Special charts for the earliest arrivals
Corrected age against the standard WHO 0–24 month charts works once a preterm baby has reached around term age. Before that — and often through NICU follow-up for very preterm infants — clinicians use dedicated preterm references such as the Fenton preterm growth chart (covering roughly 22 to 50 weeks postmenstrual age) or Intergrowth-21st standards. If your baby's clinic plots on one of those, the numbers won't line up with a WHO-based app chart, and that's expected: different reference, different question.
Percentile whiplash, preemie edition
Two chart moments that reliably alarm preemie parents but are commonly benign: the switch from a preterm chart to the WHO charts (percentiles can jump because the reference changed, not the baby), and catch-up growth — many preterm babies climb across percentile lines in the first year or two, which is the direction of travel everyone hoped for.
